Terms & Conditions

1. Introduction

These Terms & Conditions govern the provision of referral services to occupational health assessments and any other additional services provided by Sherrards Occupational Health ("Service Provider") to employers ("Client").

2. Services Provided

The Service Provider agrees to refer employees of the Client to qualified occupational health professionals for assessments. The scope of services includes:

  • Handling the initial request from the client and liaising with the third party carrying out the occupational health assessment to arrange the appointment.

  • Coordination of assessment appointments.

  • Communication of assessment outcomes to the Client.

The Service Provider can also provide additional services as requested by the Client, which will be charged on a bespoke basis in accordance with the Fee Schedule & Service Levels applicable at the time of the request.

3. Client Responsibilities in Respect of Occupational Health Referrals

The Client agrees to:

  • Provide accurate and complete information about the employee to be assessed, by completion of a referral form.

  • Obtain the necessary consents from the employee for the referral and assessment.

  • Ensure the employee attends the scheduled assessments.

4. Confidentiality

The Service Provider will maintain the confidentiality of all employee information in accordance with applicable data protection laws, including the General Data Protection Regulation (GDPR).

5. Fees

The Client agrees to pay the Service Provider for occupational health assessment referrals or any additional services requested by the Client as per the Fee Schedule & Service Levels applicable at the time of the referral or service request. Price increases will be notified to the Client in advance of them taking effect.

6. Payment Terms

Payment terms are as follows:

  • Payment is due upon presentation of our invoice.

  • Accepted payment method is via bank transfer.

7. Cancellation of Appointment Policy

The Client may cancel a scheduled assessment by providing at least 48 hours' notice to the Service Provider. For any cancellations after 48 hours before the appointment or for “failed to attend” the appointment, the following cancellation fees will be payable by the Client.

Less than 24 hours before the appointment 100% of the assessment fee
Failed to attend the appointment 100% of the assessment fee

The Service Provider reserves the right to cancel or reschedule assessments due to unforeseen circumstances, with notice provided to the Client as soon as possible.

8. Liability

The Service Provider is not liable for any outcomes of the occupational health assessments. The Client acknowledges that the assessments are conducted by independent healthcare professionals.

9. Termination

Either party may terminate this agreement with 30 days' written notice. Upon termination, the Client will pay for all services rendered up to the termination date.

10. Duration of Services

The duration of services provided under these Terms & Conditions is on an ongoing basis, subject to Clause 7 Termination. 

11. Governing Law

These Terms & Conditions are governed by the laws of England and Wales.

12. Amendments

Any amendments to these Terms & Conditions must be made in writing and signed by both parties.
